Alternate Contact

NAME: The name of the second contact person, if different from the Executive(s).

TITLE: The title of the second contact person in relation to the organization described in the record. For example: Office Manager.

ORGANIZATION: The name of the organization of the second contact person if different than that of the record itself.

PHONE: REQUIRED FIELD, MUST BE COMPLETED IF THERE IS A SEPARATE NUMBER FOR THE CONTACT PERSON DIFFERENT THAN THE OFFICE PHONE NUMBER.
Must follow the same format as for the Office_phone. Use Res to indicate if it is a home number and Bus to indicate a work phone number For Example Res 905-123-4567.
