Alternate Executive

NAME: The name of a second executive officer. This will be the president of the board if Exec Name 1 lists the chief paid staff person of the organization. This field should only be filled in if Exec Name 1 is NOT empty. This field is displayed as Executive.

TITLE: The title of the second executive officer in relation to the organization described in the record.

ORGANIZATION: The name of the organization of the executive officer if different than that of the record itself.

PHONE: Required field, must be completed for each record. Must follow the same format as for the Office phone. Use Res to indicate if it is a home number and Bus to indicate a work phone number For Example Res 905-123-4567.
