Primary Contact

NAME: Required Field, must be completed for each record.
This is the public contact person. It is the name of the main person to contact for service or information if different from the executive officers. Enter the first and last name of the individual. It is not necessary to enter titles unless they are a formal such as Dr or Rev or if requested to reduce ambiguity e.g. Mrs Pat Smith.

TITLE: Required field, must be completed for each record. Follows the Contact Name. The title of the main contact pe
rson in relation to the organization described in the record. Capitalize each word of the title. For example: Administrative Assistant.

ORGANIZATION: The name of the organization of the first contact person if different than that of the record itself.

PHONE: Must be completed if there is a separate number for the contact person different than the office phone number. Must follow the same format as for the Office phone. Use Res to indicate if it is a home number and Bus to indicate a work phone number For Example Res 905-123-4567.
