This field is displayed as Executive: and follows the Executive Name The title of the executive officer in relation to the organization described in this record. Capitalize each word in the title. For example: Executive Director.

ORGANIZATION: The name of the organization of the executive officer if different than that of the record itself. This field is displayed as Executive: and follows the Executive Name and title.

PHONE: REQUIRED FIELD, MUST BE COMPLETED FOR EACH RECORD.
Must follow the same format as for the Office phone. Use Res to indicate if it is a home number and Bus to indicate a work phone number For Example Res 905-123-4567 Res 613-555-1212 Bus 613-555-2333.
