Description (Service)Access to health care support at home, at school or in the community 
* Care Coordinators work with people in the community or leaving hospital to determine what help is needed and arrange for care 
* Care is provided in community setting such as the home, school, residential facility or CCAC Nursing Clinic 
* Information and referral to other community services when appropriate * provided through the information and referral telephone line in English or French * or online access to health care information and services in Ontario at 
Services include: 
* Children's Health Services 
* Health Care Connector Program 
* In-Home Services 
* Mental Health and Addictions School Program 
* Nursing Services in CCAC Nursing Clinics 
* Telehomecare Program 
Specialized services may include: 
* Access to assisted living and supportive housing 
* Access to respite care and convalescent care (short-term stays and adult day programs) 
* Access to Long-Term Care Homes, rehabilitation and complex continuing care beds 
* School health support services to school aged children 
* Community support for people living with cancer 
* Hospice palliative care
