Eligibility Survivor of a contributor who paid into the plan 1/3 of their contributory period, for a minimum of three years to a maximum of ten years
Application Applicant needs documents that prove the contributor's death (copy of death certificate or funeral director's statement) * applicant's age * deceased's age (usually a birth certificate) * copy of marriage certificate, or proof of having lived together (eg insurance policies or joint bank accounts)  
In person service at the Barrie Service Canada Centre, no appointment necessary * application form available at local Service Canada Centres or Funeral Homes * completed application forms and correspondence can be sent to PO Bag 2013, Timmins ON P4N 8C8 * completed application forms can be dropped off at all Service Canada Centres (staff at the centres will certify documents, but cannot confirm if correct documents have been included)
