Description

Description (Service) Program provides frail seniors with assessments and ongoing support from an interdisciplinary team to help participants stay safe and healthy in the community.

Team aims to support participants' health goals while providing comprehensive and up-to-date health information to their health care providers.

The interdisciplinary team may consist of a Nurse Practitioner, Occupational Therapist, Registered Nurse and/or Social Worker.

Topics to be discussed at appointment:
  • Overall health status
  • Possible memory changes
  • Balance
  • Mood changes
  • Cognition (thinking skills)
  • Activities of daily living

Hours Mon-Fri 8 am-4 pm
Areas Served Christian Island ; Midland ; Penetanguishene ; Springwater ; Tay ; Tiny
Eligibility Resident in Simcoe North
Application Physician referrals will only be accepted by fax from physicians in North Simcoe.
Languages English
Fees Service - Most services covered by OHIP ; None - fees may apply to non-medical services such as a Dr.'s letter confirming illness
