| Description (Service) | Provides support for individuals 16 and older living with mental health and/or addiction challenges in Simcoe County Housing and Cooperative buildings located in Bradford, Beeton and Alliston. Mobile support team provides flexible case management, housing support, system navigation and care coordination. Crisis intervention is available when required. Education and support in maintaining homes and assistance with activities of daily living is available for those who would like to receive such services. |
