| Description (Service) | Provides support for individuals 16 and older living with mental health and/or addiction challenges in Simcoe County Housing and Cooperative buildings located in Bradford, Beeton and Alliston * Mobile support team provides flexible case management, housing support, system navigation and care coordination * crisis intervention is also available when required * education and support in maintaining homes and assistance with activities of daily living is available for those who would like to receive such services |
