Residential and Community Hospice providing a supportive environment for end-of-life clients and their families. Care provided by professional staff health team and specially trained volunteers.

Our Residential Services provides 3 programs:

  • End of Life Care - provide patients who are at the end of life with holistic care, including pain and symptom management, emotional, psychological, and spiritual care. View our residence page to learn more about our amenities.
  • Pain & Symptom Management - Patients with palliative illnesses are admitted to our residence if they are having challenges with symptom management at home. Our clinical staff will work with them to provide solutions and strategies to their symptom issues so they can be as comfortable as possible when they return home.
  • Caregiver Relief - Allows caregivers of palliative clients to take a week for themselves to rest and relieve burnout while knowing their loved one is being well cared for at our residence.

Hours Community Hospice Program Hours: Mon-Fri 9 am-5 pm  
Residential Hospice: Mon-Sun 24 hours
Areas Served Adjala-Tosorontio ; Bradford West Gwillimbury ; Essa ; Innisfil ; New Tecumseth
Eligibility Program - End of Life Care:
  • DNR in place
  • Comfort care only
  • Prognosis is less than 3 months
  • Declining function or health status

Program - Pain & Symptom Management:
  • DNR in place; goals of care are comfort, not curative care
  • Progressive, life limiting diagnosis
  • Client has at least one symptom that is difficult to manage at home
  • PPS for applicants with dementia must be 40% or less

Program - Caregiver Relief:
  • DNR in place: goals of care are comfort, not curative care
  • Progressive, life limiting diagnosis
  • Prognosis is less than 12 months
  • PPS for applicants with dementia must be 40% or less
  • LTC application in place or have a place/home to return to after their stay
  • Plan of care for responsive behaviors within Matthews House abilities
Application
  • For patient referral to residential facility please call 705-435-7218 and speak with Intake Coordinator.
  • Use the Common Referral Form to detail patient information as fully as possible.
  • Fax the completed Common Referral Form and other relevant information to Matthew's House Hospice at 705-435-2755
  • Call to speak with Intake Coordinator who will confirm receipt of information
Languages French, Italian, Portuguese and Spanish Interpretation available
