| Description (Service) | Provides nursing care and personal support care * staff are certified to provide care to clients with Acquired Brain Injury and to clients with Dementia * PSW level or Nursing level geriatric training Nursing Care Services include: * palliative care * pediatric care * wound care * mental health care * post-operative care * medication management * health teaching * wound and stoma therapy advice * Nurse Continence Advisor services * physical, mental health and mobility assessments * medication and nutrition review, screenings Personal Support Care services include: * light housekeeping * meal preparation * personal care * friendly visiting * companionship and accompaniment to appointments * respite care FALLS PREVENTION: * Fall prevention assessments in the client's home * Member of Simcoe Muskoka Integrated Fall Strategy (SMIFS) |
