| Description (Service) | The Health Care Connect Program is a free service that refers Ontarians who do not have a primary care provider (family physician or nurse practitioner) to one locally who is accepting new patients * priority is given to individuals who have greater health needs * waiting period depends on availability of family physician or nurse practitioner Care Connectors are nurses who: * review health information provided upon registration with the program * follow-up with the individual with additional questions, if needed * work with local family health care providers to determine who may be accepting new patients in the community * once registered with the Health Care Connect Program an individual receives contact information for his/her assigned Care Connector The Care Connector is the main point of contact for the individual on the program and can be contacted directly by an individual for questions about the program or changes to the information they provided to the Care Connector during the registration process |
