Description (Service)

Goals of program are to increase patients' sense of control over illness and prevent hospitalization

provides medication, symptom monitoring, wellness promotion, community service awareness, education and psychosocial support for patients who have a diagnosis of schizophrenia or schizoaffective disorder who have demonstrated inadequate response treatment and/or have intolerable side effects, and need to be stabilized on new or current medications
patients will be transitioned back to the care of family physician (GP) or former psychiatrist after approximately 6 months of education and monitoring

Eligibility	Youth, Adults 16yrs and up formal diagnosis of schizophrenia or schizoaffective illness must have family physician (GP) in Simcoe County
