Description (Service)

Group program running 3-5 days per week offering assessment, stabilization, community referral and follow-up for individuals with a diagnosis of serious mental illness

groups are offered in-person and virtually
groups offered include stress and anxiety management, problem solving and communication, network building, concurrent disorders, mindfulness and DBT Skills
length of program varies from four to twelve weeks
individual counselling
family/social supportive counselling
brief transitional case management for those transitioning from the hospital to the community

Hours	Mon-Fri 8:30 am-4:30 pm
Areas Served	Muskoka District ; Parry Sound District ; Simcoe County
Eligibility	16 yrs and up Youth, Young Adults, Adults must have a diagnosis of serious mental illness
Application	Physician Referral Required (or Nurse Practitioner) - from inpatient unit, psychiatrists, Crisis Intervention Services and Emergency Department
Languages	English
Fees	None - paid by OHIP
