Description (Service) | Care Coordinators work with individuals of all ages in community settings such as home, school, residential facilities or Community Nursing Clinics to increase access to health care supports to enable them to live independently in the community and to support the safe transition home from a hospital stay Information and referral to other community services provided through the information and referral telephone line in English or French and through online access to health care information and services in Ontario at http://www.nsmhealthline.ca/ Services include: * Children's Health Services * Health Care Connect (for individuals who a looking for a doctor) * In-Home Services including personal support, physiotherapy, occupational therapy, respiratory therapy, speech and language pathology, nutritional services, social work, specialized nursing services * Mental Health and Addictions School Program * Community Nursing Clinics * Telehomecare Program Specialized services may include: * Access to assisted living and supportive housing * Access to respite care and convalescent care (short-term stays and adult day programs) * Access to Long-Term Care Homes, rehabilitation and complex continuing care beds * School health support services to school aged children * Community support for people living with cancer * Hospice palliative care |
