Description (Service) Access to health care support at home, at school or in the community  
* Care Coordinators work with people of all ages in schools, in their homes and community, and support the safe transition home from a hospital stay and arrange for care supports  
* Care is provided in community settings such as a home, school, residential facility or CCAC Community Nursing Clinic  
* Information and referral to other community services when appropriate * provided through the information and referral telephone line in English or French * or online access to health care information and services in Ontario at www.thehealthline.ca  
 
Services include:  
 
* Children's Health Services  
* Health Care Connect (for individuals who a looking for a doctor)  
* In-Home Services  
* Mental Health and Addictions School Program  
* Nursing Services in a CCAC Community Nursing Clinic  
* Telehomecare Program  
 
Specialized services may include:  
 
* Access to assisted living and supportive housing  
* Access to respite care and convalescent care (short-term stays and adult day programs)  
* Access to Long-Term Care Homes, rehabilitation and complex continuing care beds  
* School health support services to school aged children  
* Community support for people living with cancer  
* Hospice palliative care

Areas Served Grey County - eastern part ; Muskoka District ; Simcoe County - south to Cookstown
Boundaries All of District of Muskoka * most of Simcoe County (south to just south of Hwy 89) * small portion of eastern Grey County, including Maxwell and Feversham
Eligibility People of all ages who are facing the challenges of age, illness, injury or disability
Application Call * no referral required for most services * referral required from school for in-school services
Languages English ; French - services can be arranged in French ; The public website supports the translation of content to over 90 languages
Fees None - CCAC services are covered by OHIP
