Description (Service) Supports for older adults and individuals with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services  
 
Services include: 
* in-home health services and community based programs to individuals of all ages  
* long-term care program includes five Adult Day Programs - Barrie, Collingwood, Orillia, Beeton and Bracebridge 
* in-home respite and enrichment program that is available to individuals with a diagnosis of Alzheimer Disease or related dementia  
* coordinate Home At Last (HAL) Program and SMART Seniors Maintaining Active Roles Together Program  
 
VON Canada Simcoe County Site is a member of the Simcoe Muskoka Integrated Fall Strategy (SMIFS) and member of the North Simcoe Muskoka Community Support Services Coalition

Hours Service: Mon-Sun 24 hours * Office: Mon-Fri 8:30 am-4:30 pm * Alzheimer Respite Services: scheduled visiting times, call for information
