Description

Description (Service)

Supports for older adults and individuals with disabilities. Helps people remain independent in their own homes for as long as possible. Referral to other community services.

Services include:

  • in-home health services and community based programs to individuals of all ages
  • long-term care program includes five Adult Day Programs - Barrie, Collingwood, Orillia, Beeton and Bracebridge
  • in-home respite and enrichment program that is available to individuals with a diagnosis of Alzheimer Disease or related dementia
  • coordinate Home At Last (HAL) Program and SMART Seniors Maintaining Active Roles Together Program
