  • Age Requirement 60 yrs and up
  • All patients must be registered with the Barrie Family Health Team Doctors/Nurse Practitioners
  • individuals living with memory loss
  • individuals age 75 and over with complicated medical conditions preparing for major surgery
Application Referral from a Family Health Team Physician or Nurse Practitioner
