Help indigenous patients with cancer access health care support and services at home

co-ordinates traditional and non-traditional community resources and healing
counselling and support before, during and after clinical appointments
liaison/support between the care team, patients and families
advocacy on behalf of patients
assistance with care planning
advocacy for end of life care focused on the individual's goals and wishes
