Description (Service) Free and voluntary (with client consent) program providing home visiting services to pregnant individuals and families with children from birth to school transition who may benefit from additional support

Home visiting team works in collaboration with the client and family to address goals and provide support for:
  • Healthy pregnancies
  • Infant and young child feeding
  • Healthy growth and development
  • Adjustment to parenthood
  • Parental health and well-being
  • Healthy relationships
  • Child and family safety
  • Accessing community supports

  • Women and their families in the prenatal period
  • families with children from birth until transition to school requiring additional support

  • Referral - can occur prenatally or following post-partum by either self-referral or by health care or social service provider with client consent
Languages English; French - Reception, Programs; Other - Service in additional languages can be arranged
