| Description (Service) | Group exercise classes 2 times/week for 12 weeks with a focus on balance, coordination, upper and lower extremity strengthening * each class is 2 hours, with time dedicated to exercise, cognitive stimulation and education * delivered by Registered Kinesiologists with access to a SGS Physiotherapist Offered in partnership with Barrie Community Health Centre, Couchiching Family Health Team and the North Simcoe Family Health Team |

| Eligibility | Frail Older Adults (Clinical Frailty Scale 4-6) * must have medical clearance to participateand must meet the following criteria: * recent loss of functional ability, including decline in one or more ADLs/ IADLs related to medical condition or life event * at risk for admission into long-stay LTCH bed or hospitalization, as result of the functional loss * has potential to regain some/all of the functional loss through participation in the classes * has cognitive ability to participate/follow instructions * tolerance to participate and willingness/ability to attend classes Priority is given to those meeting the eligibility criteria and being discharged from Acute Care of the Elderly (ACE) beds, Complex Continuing Care (CCC) beds, Convalescent Care Program (CCP) beds and patients of the SGS program |
