| Application | * Call the Indigenous Independent Living Care worker (IILC) at 705-818-3080 or contact by email at iilc@banac.on.ca * referral form can be filed out through self-referral, partnering agency, community organization or from a family physician * form can then be directly submitted to the IILC worker to confirm the eligibility * IILC worker will personally meet with the individual that is new to the program, to fill out the intake and assessment forms to identify any needs or requirements for the services provided, to develop a plan of care and to bridge the new connection between client and IILC worker. |
