Description (Service) Family Physicians, Mental Health Counsellors and Psychiatrists work together to help individuals, couples, and families deal with issues such as depression, anxiety, stress, grief and loss, relationship problems, emotional eating * provides a range of services to enrolled patients of Family Health Team physicians * Mental Health Counsellors are qualified professionals who work in a short-term, focused, and confidential manner to find solutions * most people work with their counsellor for approximately 6-8 sessions * work collaboratively with the physician to provide care and meet needs * services include Individual, family, and/or marital counselling, Psychiatric consultation, linkages to community resources

Application Referral from Family Health Team Physician only * intake assessment appointment scheduled upon receipt of referral * wait time after this appointment before a counselor is assigned will be based on urgency
