Description
Description (Service) | Helps make the transition from hospital to home. Provides transportation and the services of a Home at Last (HAL) Attendant (PSW). Once at home the HAL Attendant can provide assistance with groceries, medication and medical supplies pick up, light housekeeping, laundry and meal preparation. The HAL Attendant will provide the client with a package of community support services. Followup within 24-48 hours by HAL supervisor. |
