Description (Service) Drivers in Ontario convicted of impaired driving or multiple administrative roadside suspensions, must complete this mandatory program * administered by the Centre for Addiction and Mental Health on behalf of the Ministry of Transportation of Ontario and offered by community service providers in a variety of locations across the province * in the York, Simcoe, Muskoka region it is offered in Aurora, Barrie, Bracebridge and Parry Sound

Eligibility Those who have been convicted of impaired driving or roadside suspension warn range drivers with more than one suspension
Application Register online, download and print the registration from website or call to have a package mailed or faxed * you will need your credit card number and driver's license number * fastest way to register is online * physical form can be mailed, faxed (credit card only) or scanned and sent by email (credit card only) * only way to register using a money order or certified cheque is by mail * register as soon as possible after receipt of suspension notice * no walk-in service
