Description (Service)

10 bed residence at Hospice Simcoe provides care to individuals who living with a palliative diagnosis, supporting them and their families with dignity and comfort in a home-like environment

  • staffed twenty-four hours a day by a professional palliative care team and trained compassionate volunteers
  • spacious private rooms enable family members to stay together
  • pain and symptom management

Eligibility Children, Youth, Adults; Older Adults, 4 yrs and older
Application Assessment is required by North Simcoe Muskoka Home & Community Care (NSMHCC)
  • Referral request must go to Home and Community Care (HCC), phone: 705-721-8010
  • The request can come from a physician, client or substitute decision maker for the client
  • HCC will complete an assessment and referral prior to admission to Hospice Simcoe, admission is based on a triage assessment tool that identifies the person most needing an admission
