Description

Description (Service) Provide information, education and support for persons living with dementia and their care partners. The following programs and services are offered:
  • Navigation (connects individuals newly diagnosed and/or who have not accessed other support / community services)
  • Supportive Counselling
  • Care Partner Support Groups
  • Education programs for persons living with dementia, their care givers, professionals and the general public
  • Social, Recreational and Exercise programs
  • Resource Library & Information packages
  • Activity Kits

Programs are delivered throughout Simcoe County.

Members of the public can self refer to the programs simply by contacting the office. A short intake will follow. Community partners and family physicians can refer clients to use using the First Link Referral form First Link Referral

Hours Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm

Summer Hours 2022: Jun 27-Sep 2: Mon-Thu 8:30 am-12 noon, 12:30 pm-4:30 pm * Fri 8:30 am-12 noon, 12:30 pm-2:30 pm

Areas Served Simcoe County
Eligibility People with Alzheimer's disease and other dementias and care partners.
Application Self referrals or referrals from Health Care Professionals. Referral form is available on website. Call, email or drop-in to access general information and/or to start the process of accessing the services. Appointments are required to speak with a Family Support Coordinator.
