Description
| Description (Service) | Provide information, education and support for persons living with dementia and their care partners. The following programs and services are offered:
Programs are delivered throughout Simcoe County. Members of the public can self refer to the programs simply by contacting the office. A short intake will follow. Community partners and family physicians can refer clients to use using the First Link Referral form First Link Referral Special Events and Fundraising Activities

For medical emergencies, contact 9-1-1. |

| Application | Self referrals or referrals from Health Care Professionals. Referral form is available on website. Call, email or drop-in to access general information and/or to start the process of accessing the services. Appointments are required to speak with a Family Support Coordinator. |
