Description (Service) | Provide information, education and support for persons living with dementia and their care partners. The following programs and services are offered:
Programs are delivered throughout Simcoe County. Members of the public can self refer to the programs simply by contacting the office. A short intake will follow. Community partners and family physicians can refer clients to use using the First Link Referral form First Link Referral Special Events and Fundraising Activities

Hours | Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm Summer Hours 2022: Jun 27-Sep 2: Mon-Thu 8:30 am-12 noon, 12:30 pm-4:30 pm * Fri 8:30 am-12 noon, 12:30 pm-2:30 pm For 24/7 dementia advice and support during the holiday season, contact Telehealth Ontario at 1-866-797-0000. For medical emergencies, contact 9-1-1. |

Eligibility | People with Alzheimer's disease and other dementias and care partners. |
Application | Self referrals or referrals from Health Care Professionals. Referral form is available on website. Call, email or drop-in to access general information and/or to start the process of accessing the services. Appointments are required to speak with a Family Support Coordinator. |
