Description (Service) Provides education and services for the prevention, management and treatment of diabetes  
  • Provides case management and screening services 
  • Provides one-on-one or group consultation on monitoring blood sugar levels and medications 
  • Offers basic foot care  
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes  
  • Advises on developing and implementing an approach to care and management of diabetes including healthy eating, exercise, and other lifestyle choices  
  • Assists individuals in finding appropriate services 
  • Coordinates regular awareness activities  
  • Provides referrals to other health care professionals
