| Description (Service) | Medical care team focused on family and community health * coordinates patient care in order to assess and treat illness * team includes physicians, nurses, nurse practitioners, registered dietitians, pharmacists, respiratory educators, diabetes educators, mental health counsellors, lactation consultants and social workers Programs and services include: * Diabetes Program * Lung Health Program * Aging Well Clinic * Smoking Cessation Program * Prenatal & Well Baby * Telemedicine * LINKS Team * School Success Program * Breastfeeding Services * Chiropody Services * Mental Health Services * Nurse Practitioner Services * Pharmacist Services * Nutrition Services * Occupational Therapist Services Multiple classes and group sessions are offered – visit www.barriefht.ca for details These services are available to all area residents who are rostered with a Barrie and Community Family Team physician * the Prenatal Well Baby Clinic provides prenatal and well baby care for women and children (up to the age of 6 years) without a family doctor in the Barrie area |
