| Description (Service) | Services include: * nursing services such as Wound care * IV medication administration and teaching, * PICC line and Portacath care * Chemo disconnects * Health Teaching, Injections * Symptom management * Ostomy care and teaching, * Catheter care * Nephrostomy care * Drain care * G Tube * Physical Assessments etc. All patients attending the NSM LHIN community nursing clinic will be assessed and provided with individualized instruction on self care and self-management supports Anyone using a mobility aid such as a cane, walker or wheel chair should bring the aid with them |
