| Application | Presently no doctor available. Call or email to request appointment, leaving name and phone number so that staff can call to book appointment. Have OHIP card number ready when calling to make appointment. Staff will also ask for address, date of birth and name of family physician. Recommended to call as early in the day as possible to obtain an appointment time. Approximate time frame of appointment will be given and physician will call prior to visit to ensure patient is home. |
