Description (Service) Nurse supervised in-home Personal Support Worker (PSW) care for individuals affected by dementia, Alzheimer’s or Acquired Brain Injury as well as palliative home care 
 
Services provided include: 
  • Arranging transportation to medical appointments, activities  
  • Assistance with activities of daily living  
  • Walks and exercise (medically approved) 
  • Personal hygiene and foot care 
  • Meal planning, preparation, grocery shopping and light housekeeping 
  • Companionship  
  • Live-in care  
  • Behavior management  
  • Care planning  
  • Case management, coordination with LHIN 
  • Cognitive stimulation, Reminiscence therapy and Memory assistance care  
  • Dementia care  
  • Medication reminders and recording  
  • Overnight and respite care  
  • Rehabilitation (medically approved)  
  • Restorative care  
  • Palliative care 
  • Supplementary care at long-term care homes, retirement residents  
  • Supplementary palliative care at home, in hospital or hospice

Hours Office: Mon-Fri 9 am- 5 pm * Service: Mon-Sun 24 hours
