Description

Description (Service) Community Health Centre staffed by:
  • Doctors
  • Nurse Practitioners
  • Registered Practical Nurses
Offers:
  • Comprehensive primary care which includes regular check-ups, chronic disease management, immunizations, pre- and post-natal care
  • Tobacco Cessation Services: help to quit smoking tobacco, free nicotine replacement therapy (nicotine patch, gum, lozenge, inhaler, mouth spray) available
  • Foot Care: must be referred by a health care provider; service is for people without extended health coverage
  • Lab Services available to anyone: blood work, B12 & depo provera injections, (do not need to be a rostered patient)
  • Nursing Services available to rostered patients: ECGs, acute illness care, injections, immunizations, wound care, blood pressure monitoring, baby weight checks

SEGCHC also offers the following programs and services:In Markdale:
  • free open gym with various equipment such as treadmills, elliptical, stationary bikes and free weights
  • skate exchange - offers free ice skates (delivery to Dundalk is available).

Hours Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm

Eligibility Unless otherwise noted, programs and services are available to all community members
  • waitlists are in place for allied health services and priority is based on condition
  • must enroll to have a Family Doctor or Nurse Practitioner
  • must have a referral from health care provider for physiotherapy
Application Submit a form called "Request for a Family Doctor or Nurse Practitioner" to have primary care at the Community Health Centre.

Walk in or call to register or inquire about community services and programs.

Volunteers need to complete a volunteer application form and a criminal reference check/vulnerable sectors screen.
Languages Interpretation services can be arranged in advance, and sometimes on-demand if interpreter is available.
