| Description (Service) | Community Navigators respond to referrals from health care providers and paramedics. These health care professionals send referrals. Navigators complete outbound calls to the referred individuals with guidance as how to connect them to assistance that will support their Social Determinants of Health (eg. housing, income, education, social connection etc.). |
