| Description (Service) | Telephone services provided include: * Palliative support and guidance to Health Care Providers and Families through consultation to confidently care for patients within their location of choice when and wherever possible across the North Simcoe Muskoka region * Healthcare providers, who treat patients at home in the community, in Long Term Care Homes, Retirement Homes and in the acute sector have the option of receiving secondary palliative consult support, from Hospice Palliative Care Nurse Consultants as an alternative to relocating patients and their families to urban areas or other healthcare facilities for palliative specialist care * Offers Palliative Care education and consultation to service providers providing end-of-life care |
