Description (Service)

Supports for people living with pre-diabetes and diabetes, including adult Type 1 or Type 2, gestational diabetes or Type 1 or Type 2 with pregnancy

Services include:

one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise and other lifestyle choices
certified diabetes educators on staff including registered nurses and dieticians
support and management for individuals using insulin pumps including ADP approval through OTN consultation appointments

Hours	Mon-Fri 8 am-5 pm
Areas Served	Clearview (Simcoe County) ; Collingwood ; Wasaga Beach
Eligibility	Ages: 18 year(s) and up Patients of non Family Health Team physicians are welcome 18 year(s) and up
Application	By appointment. Physician referral. Self-referrals welcome for basic education support.
