Hours	Office: Mon-Fri 8 am-5 pm After Hours Clinic: Mon-Fri 5 pm-8 pm (arrive before 7:30 pm), Sat 9 am-3 pm (arrive before 2:30 pm)

Eligibility	Individuals with a family physician associated with Couchiching Family Health Team No appointment or referral necessary Read signage at entry to building and observe all protocols in place
Application	After Hours Clinic: Arrive half hour prior to closing with valid Heath Card Visit Website for more information Preventing and managing diabetes: A referral is required from your physician or from a member of the Diabetes team
