Description (Service)

Provides care for patients with physical or mental health concerns who reside in Collingwood, Clearview, Blue Mountain (L9Y) or Wasaga Beach who do not have a family doctor or nurse practitioner anywhere in Ontario.

Patients will be seen by a nurse practitioner.

Due to the limited number of appointments available at this clinic, concerns that require extensive follow-up will not be seen.

This clinic now has a waitlist for intake appointments to be registered at the clinic. Please follow the process outlined in the Application Section.

Service Details

Hours	Mon, Tue, Fri 8:30 am-4:30 pm * Wed, Thu 8:30 am-3 pm, by appointment.
Areas Served	Clearview (Simcoe County) ; Collingwood ; Wasaga Beach
Eligibility	Patients who live in Collingwood, Clearview, Wasaga Beach and those in The Blue Mountains with L9Y postal code and do not have access to a family doctor or nurse practitioner anywhere in Ontario. Patients with a physician anywhere in Ontario are unable to access service at the Shoreline Health Clinic.
Application	Individuals who have already registered with the Shoreline Health Clinic can book an appointment by clicking here Anyone who has not registered will need to be placed on a waitlist for an intake appointment. Note: Anyone who already has a physician in Ontario, but is looking for a local physician can complete the Primary Care Waitlist form. See separate listing
