| Description (Service) | Provides care for patients with physical or mental health concerns who reside in Collingwood, Clearview, Blue Mountain (L9Y) or Wasaga Beach who do not have a family doctor or nurse practitioner anywhere in Ontario. Patients will be seen by a nurse practitioner. Due to the limited number of appointments available at this clinic, concerns that require extensive follow-up will not be seen. |

| Eligibility | Patients who live in Collingwood, Clearview, Wasaga Beach or Blue Mountains (L9Y) and do not have access to a family doctor or nurse practitioner anywhere in Ontario. Patients with a physician anywhere in Ontario are unable to access service at the Shoreline Health Clinic |
| Application | Individuals who have already registered with the Shoreline Health Clinic can book an appointment by clicking here Anyone who has not registered will need to be placed on a waitlist for an intake appointment.
