Description
| Description (Service) | A service for youth and young adults with developmental disabilities, which will assist them in the transition process from school to life as adults in the community. Supports people to be able to make plans for future life directions most appropriate to their needs and abilities and to move on to the next phase of their life upon leaving the program. This is achieved through the development of identified skills and exposure to a wide range of appropriate opportunities that can best support the unique needs of the individual. |
