Description

Description (Service) Healthy living series for individuals living with Diabetes, Heart Disease, Arthritis, Chronic Lung Disease, Asthma, Fibromyalgia, Chronic Pain and other long-term illnesses.

Two separate 6-week programs: Living a Healthy Life with Chronic Pain and Living a Healthy Life with Chronic Conditions.

Self-management programs cover topics such as:
  • managing symptoms
  • dealing with emotions
  • exercise and nutrition
  • pain management
  • breathing techniques and muscle relaxation
  • communication and problem-solving
  • medication usage
  • treatment decisions
  • planning for the future
  • goal setting and more
  • sleep management
  • understanding acute and chronic pain

Meet and learn from others with chronic conditions and/or chronic pain. Each week the group learns skill to better manage symptoms, the challenges associated with this and ways to improve their quality of life. Varied times and locations throughout North Simcoe and Muskoka.

Support persons are also welcome.
