Description

Description (Service) A team of mental health professionals who provide treatment, rehabilitation and support for people living in the community who have complex needs related to severe and persistent mental illness and who are frequent users of mental health and community services.

The team provides counselling, case management and housing support.

Key services are:

  • assistance with activities of daily living
  • medication assistance
  • skills training
  • vocational support
  • family education and support

Hours Program hours: Mon-Sun 8 am-8 pm

Office: Mon-Fri 8 am-4 pm
Areas Served Owen Sound
Eligibility Ages: 16 year(s) and up Resident of Grey or Bruce Counties with a serious mental illness. Clients must live within 30 km of Owen Sound
