Offers a range of personalized medical and non-medical home care services tailored to meet the unique needs of each individual, communication between family members and caregivers is emphasized to ensure everyone is aligned in the care process

• Trained caregivers assist with daily activities, personal care services including eating, grooming, dressing, bathing, incontinence, cognitive impairment and mobility
• Home help including light house keeping, meal preparation and grocery shopping
• Companionship, and support for those living with chronic illnesses or recovering from surgery
• Care for individuals with dementia and Alzheimer's disease
• Hospice Support and respite for family members
• Nurse Directed Care to help with medication administration, wound care, glucose monitoring, insulin administration, oxygen therapy, home dialysis assistance

Minimum of 2 hrs per visit and 10 hours per week
Veteran Affairs Canada (VAC) Health Identification Cards accepted
