Description (Service) The Collaborative is a group of LHIN-funded agencies providing a range of community services that enable the residents of North Simcoe and Muskoka to live safely and independently in their homes and communities 
 
These services may include Meals on Wheels, Home Support and Palliative Care  
 
Some agencies provide services to specific populations - such as Individuals with a physical disability, vision loss, hearing loss, acquired brain Injury

Eligibility Ages: 16 year(s) and up 
 
Membership is open to all agencies funded by a Local Health Integration Network to provide community support services within North Simcoe County and the District of Muskoka 
 
Member agencies provide services to adults, 16 years and older
