| Description (Service) | Offers specialized assessment and support for individuals experiencing mental health difficulties Treatment plans may include: * individual psychotherapy * addictions counselling * groups (educational and/or therapeutic) * support coordination * counselling, education and support for individual family members * specialized geriatric services (assessment and referral) * seniors intensive case management * Behavioral Support Ontario (BSO) Seniors Crisis service * psychiatric resources * court support services * Rapid Access Addiction Medicine Clinic (RAAM) Integrated Crisis Response Team Crisis Support Workers work with individuals to determine an immediate and short-term plan to help stabilize the crisis, and work with other community resources to prevent the re-occurrence of crisis and develop capacity for service pathways to meet individuals' needs. |
