Hours | Community Paramedics are available 24 hours a day/7 days per week Mon-Sun 7 am-10 pm (in-person appointments) Mon-Sun 10 pm-7 am (virtual only) |

Eligibility | Patients who require additional assistance while living in their home, including high-risk clients living with complex medical issues such as diabetes, COPD and CHF |
Application | Email or fax form Microsoft Word - Community Paramedicine Referal blank form v.2.1 (COVID) (headwatershealth.ca) Referrals from patients, family members or service providers will be reviewed upon receipt to determine eligibility for program. Community Paramedics will make initial contact with the patient within 24 hours after receiving a referral to discuss outcome. |
