Aims to reduce 911 calls, emergency department visits and hospital admissions. Services include: * in-home assessments * safe home assessments * scheduled wellness checks * remote patient monitoring * direct communication with primary care providers * diagnostic tests |

Eligibility | Patients who require additional assistance while living in their home, including high-risk clients living with complex medical issues such as diabetes, COPD and CHF |
Application | Accepts referrals from patients, family members or service providers * will make initial contact with the patient within 24 hours after receiving a referral to determine if the patient will benefit from the service |
