Description
Description (Service) | Offers tele-intervention, home visiting, early intervention and prevention services and supports to assist families with infants and toddlers with established risks, biological risks and psycho-social risks of delays in development or disability to attain an optimal level of development. Parents are recognized as primary participants in this process. All services and supports are family-centred. Services include:

Eligibility | Ages: 6 year(s) and under Children at risk of delays in development and children with identified special needs. Children are eligible for services until school entry. |
Application | Call to make a referral or request a consultation. Informed parental consent is required. |
