Description

Description (Service)

Provides home care rehabilitation services in communities throughout Bruce and Grey counties. Services are available by referral from the local Ontario Health atHome branch, as well as on a private basis.

Therapy services available include:

  • Physiotherapy - in home and clinic
  • Occupational Therapy
  • Dietitian services
  • Social Work - including counselling, mediation and /or program development
  • Speech-language services.
  • Motor Vehicle Accident Therapy

Service Details

Hours Home Care Office: Mon-Fri 8:30 am-4:30 pm, services available at other times, if required.
Clinic: Mon-Fri 9 am-4 pm
Areas Served Bruce County ; Grey County
Eligibility Anyone requiring clinic physiotherapy services. OHIP coverage for those 19 years and under, 65 years and older, ODSP or Ontario Works recipients and people hospitalized for at least one night due to an injury.
Application For services through Ontario Health atHome referral: Call Ontario Health atHome South West Intake. For private services: Call Closing The Gap office.

No referral required for private pay services. Medical referral required for OHIP coverage. Medical referral may be required for third-party insurance.
